• Complete the Research Membership Form if you are U of T faculty member primarily involved in diabetes-related research. Research members are required to hold their primary faculty appointment at the level of assistant, associate or full professor.
• Complete the Care and Education Membership Form if you are U of T faculty or a health professional primarily involved in diabetes care or education.
