The Centre for Complex Diabetes Care provides intensive short term care to patients with complex diabetes needs including mental health, barriers to care and complexities requiring case management. Collaborative, case managed care is provided by an interdisciplinary team including Endocrinologists, Nurse Practitioners, Nurses, Dietitians, Social Workers, a Pharmacist, and a Chiropodist. The CCDC serves the Central Local Health Initiative Network and is an initiative of the Ontario Diabetes Strategy.
